St. Luke’s Health Partners (SLHP) is an Accountable Care Organization (ACO). ACOs are groups of doctors, hospitals, and other health care professionals that work together to provide coordinated, high-quality care to their Medicare Fee-for-Service beneficiaries. Coordinated care helps ensure beneficiaries, especially the chronically ill or those who are hospitalized, get the right care at the right time, and avoid unneeded duplication of services such as lab tests and x-rays. SLHP ACO is a non-profit organization that has agreements with doctors and professionals.

Notice of Program Termination. There are No Changes to your Care.
Beneficiaries recently received a letter about St. Luke’s Health Partners participation in the ACO REACH program. We understand that the required wording of this notice may have been confusing, so we want to reassure you that your health care and coverage are not changing.
What’s Not Changing
- You will continue to have access to St. Luke’s doctors, specialists and care teams—just as you do today.
- Your Medicare coverage remains the same.

No action is required from you. You do not need to change providers because of this notification.
Why You Received the Letter
This notice is required by the Centers for Medicare & Medicaid Services (CMS) when participation in a program changes. St. Luke’s Health Partners will no longer participate in ACO REACH after December 31, 2025. This change does not affect how you receive care at St. Luke’s or your Medicare coverage.

Participating and Preferred Providers
There are no joint ventures between or among the ACO and any of its ACO Preferred Providers and Participant Providers.
Definitions for Participating and Preferred Provider Types
The ACO REACH model features two categories of Medicare providers. The two primary categories are Participants and Preferred Providers.
- Participating Providers are the core group of providers that form the ACO. Participants are typically primary care physicians. They are bound to specific requirements under and are responsible for reporting quality and committing to beneficiary care improvement. They are used for beneficiary (patient) alignment.
- Preferred Providers are typically specialists/facilities/ancillary services which are defined as "in-network" and qualify for beneficiary enhancement and waivers. Preferred Providers contribute to ACO goals by extending and facilitating valuable care relationships. Preferred providers are not used for beneficiary alignment.
