Care Management
Care management provides personalized support and information for patients and their families as they make important choices about treatment, health care preferences, and personal goals. Our care management team consists of registered nurses, licensed social workers, registered dietitians, pharmacists, respiratory therapists, outreach coordinators, and community health workers all working together to support our patients.Care management services are provided at no cost to you. However, you may be responsible for co-payments and other costs, based on your health plan benefits, for office visits or services that may be recommended through care management.
Eligibility and Enrollment
Care management is one of the benefits of your individual health plan and your provider, being a member of the St. Luke’s Health Partners network. You may be eligible for the program due to a chronic illness, a recent hospitalization, or evidence of a progressive illness for which you may require additional support. Your care advisor can provide additional information about how you became eligible for these services.
To enroll, call your care advisor directly at the phone number on the welcome letter you received. Or, call (208) 493-0332 and someone will return your call within one business day.

What if I already have a primary care provider that I see routinely? Do I still need care management services?
Yes! Your care advisor works in partnership with your provider to ensure you are informed of all treatment options. Your care advisor will help you navigate treatment options and your health plan coverage.
Who will have access to my personal medical information?

Can I disenroll if I no longer want to participate?
Yes, this is a voluntary program. You can disenroll at any time by notifying your care advisor directly or calling (208) 493-0332.
Will I know who my care advisor is?
Yes, your care advisor’s name and credentials are on the welcome letter you received. Your care advisor will also identify themselves at each telephone call and in all mailed communications.
How do I request a different care advisor?
If you wish to change to a different care advisor, you may do so by calling (208) 493-0332. Someone will call you back within one business day and assign you a new care advisor.
What can I expect from my care advisor?
Care advisors are partners in your healthcare journey. You can expect them to offer you courtesy and respect; and communication that you can understand, with translation support as needed.
What is expected of me?
To get the most out of your individualized care management program you will be expected to:
- Share information to support the development of your care plan
- Follow the care plan developed by you with support from your care advisor, or let your care advisor know if you are unable to follow your plan
- Notify your care advisor and primary care provider if you need to discontinue for any reason
What if I am unhappy with the program?
You can file a complaint, or provide comments by phone, email, or by mail. Upon receiving a complaint, a response will be provided within five business days, and you will be informed of any investigation and its outcome.
