2017 Core Quality Set

St. Luke’s Health Partners Clinical Integration Committee is pleased to announce the 2017 Core Quality Measures Set. These six quality measures will be the focus for quality improvement efforts for primary care in 2017. Clinical Practice Guidelines

Clinical Practice Guidelines (CPGs) provide evidence-based information to guide effective, consistent treatment and management for common diseases. CPGs summarize current medical literature and are tools intended to help network providers and their clinical teams decrease variation in practice and improve continuity of care, thus improving the Core Quality Set.
